Test Driven Development is all you need
Coming to this a bit late but the TDD point has aged brilliantly. I've seen the same thing; without tests as guardrails the subagent outputs drift fast. The two-stage review approach (spec compliance + code quality as separate passes) turned out to be another piece of the puzzle.
